1. When you download this form, you will have two pages, Side A and Side B.
2. On Side A, list all the courses you have completed for your major, and verify with your major adviser whether or not they meet your major requirements.
3. On Side B, together with your major adviser, list your remaining major requirements for the adviser's verification and approval.
4. After both you and your adviser have signed and dated this form, turn in the completed Goldenrod to the Service Counter at Hawaii Hall 108.

INSTRUCTIONS

1. When you download this form, you will have a total of four pages because Copy 1 is submitted to the college you're entering, and Copy 2 is submitted to Admissions and Records. These pages are duplicates, but you are responsible for submitting all four completed pages.
   
2. This form is used to transfer into the Colleges of Arts and Sciences from other UHM schools or colleges, and/or to declare, change, add, or drop an Arts and Sciences major.
   
3. You fill out the first page of Copy 1 and Copy 2.
   
4. Take the form to Hawaii Hall 108 to submit for approval if you are presently in another UHM school or college (i.e., Engineering, Architecture, Education, etc.) and now wish to transfer into the Colleges of Arts and Sciences.
   
5. If you are presently in another UHM school or college and wish to transfer into the Colleges of Arts and Sciences and also wish to declare an Arts and Sciences major, fill out the second page of Copy 1 and Copy 2 with the major adviser. 6. If you are currently enrolled in the Colleges of Arts and Sciences and wish to declare, change, or add a major, go to the appropriate department and fill out the second page of Copy 1 and Copy 2 with a major adviser. If you wish to drop a major you do not need to obtain the department's approval, so fill out the first page of both copies and leave the second page blank.

INSTRUCTIONS

1. This form is used to certify completion of an academic minor or verification of courses needed to complete an academic minor.
2. Fill out the top part of the form, indicating your biographical information and the courses that you have taken to fulfill minor requirements. Identify courses in progress with an asterisk (*). Sign and date in the spaces provided.
3. Take the form to your departmental minor adviser for review, verification, and signature.
4. Take the form to your departmental major adviser for certification that none of your minor courses are being used to satisfy a requirement in your major.
5. For Arts & Sciences students: Attach the form to your Goldenrod, or turn the form in at the A&S Student Academic Services Counter (Hawaii Hall 108), or bring the form to your GRAD Session.

NOTE: If you do not submit your Certification of Academic Minor and fulfill all minor requirements before graduation, you will not be granted the minor. You will not be able to file for and/or complete minor requirements after graduation.
