Certification of Academic Progress: Forms and Memos

Forms related to student academic progress and standing often require the signature of an academic advisor.  Some forms can be dropped off for review and clearance, and picked up by the student 3 to 5 business days later.  Others may require an in-person appointment.  Listed below are the types of requests we receive most often, and the procedures for having them filled.

Forms provided by our office, if accessible, can be downloaded by clicking on the form name. You will need the Adobe Acrobat Reader to view and print them.  NOTE the turnaround time for drop-offs: 3 to 5 business days. Due to the large amount of review requests received daily, we are not able to accommodate 'Rush' requests.

 

GENERAL VERIFICATION OF PROGRESS
Who: UHM Arts & Sciences majors who need verification of academic progress or projected graduation date for employers, scholarships, graduate programs, etc.

You need to have completed either a GRAD or a JUMP Session. Fill out a Request for Written Verification Form at QLC 113 ; it will be ready for pick up within 3 to 5 working days.  If you want it mailed instead, you must provide a pre-addressed stamped envelope.  You can also request that it be faxed; provide the name and fax number on your request.

 

FINANCIAL AID RELATED

UHM Community College Enrollment Memo - appointment needed
Who: UHM Arts & Sciences students receiving financial aid, with part-time registration at UHM and concurrent enrollment on other campuses (ex. 9 credits at UHM plus 3 credits at KCC)

Download and fill out the form before making an appointment with a college advisor to have it verified and signed.  (956.8755 ext.1 for appointments.) If you have any questions regarding the form, please contact your Financial Aid Counselor at 808.956.7251, QLCSS room 112.

INTERNATIONAL STUDENTS (J-1, F-1 Student Visa)

Request for Less Than Full-Time Enrollment - appointment needed
Who: international students who, for reasons of health, academic difficulty or completion of remaining requirements, would like to take a lighter course load

International students may be allowed a reduction in courseload depending on circumstance. See an International Student Services counselor in QLCSS 206, 956.8354 for information on eligibility. If given the form, make an appointment to meet with an Arts & Sciences academic advisor by calling 956.8755 ext.1. Be sure to bring your form and any supporting documentation, if needed, with you.

Optional Practical Training (OPT) - appointment needed
Who: International students remaining in the U.S. to work after graduation

See an International Student Counselor in QLCSS 206, 956.8354 for information on eligibility.  If given the form, make an appointment with an Arts & Sciences academic advisor to verify your graduation date and relation of the intended training program to your field of study. 956.8755 ext.1

 

NSE (National Student Exchange)/STUDY ABROAD

Who: UHM Arts & Sciences majors who are applying to study in another state through the National Student Exchange Program, or overseas through the UHM Study Abroad Program.  If you are applying to study in a foreign country through a NON-UHM program, please come to HH108 for an OVERSEAS LEAVE OF ABSENCE form.

Satisfactory Academic Progress Form for Study Abroad
Who: UHM Arts & Sciences majors applying to a Study Abroad program, and who wish to be considered for financial aid while abroad

You receive this form in your Study Abroad package. Fill in your contact information and drop it off at QLC 113. An advisor will review your academic status, sign if approved, and forward it directly to the Financial Aid office within 3 to 5 working days.

Verification of Course Transferability and Applicability to UHM Requirements
Who: UHM A&S majors who need to show that the courses being taken through NSE or Study Abroad will apply to degree requirements here.

Under certain circumstances The UHM NSE or Study Abroad program may require verification that courses a student intends to take will a) be accepted in transfer by the UHM Admissions Office, and b) fulfill necessary requirements for the student's degree.  

  • Identify your remaining requirements by completing a JUMP or a GRAD Session, and meeting with your major department advisor.
  • Choose courses which appear applicable to your remaining requirements.
  • Check for transferability of intended courses in the Admissions Office's Transfer Credit Database: www.hawaii.edu/admrec  click on "Transfer Credit Search."
  • For any course not found in the database, you must make an appointment to have the course description reviewed by the course department's undergraduate advisor. Request a written evaluation, which should indicate whether the course is a) acceptable in transfer, b) equivalent to a specific course at UHM or c) acceptable as an elective, and d) if it is considered to be at the non-introductory level.
  • If you need an A&S advisor's signature, make an appointment by calling 956.9755 ext. 1.  Bring all of your evaluation memos with you.  Otherwise, turn in your evaluations at the Admissions Office, QLCSS room 1.

 

ROTC CADETS (A&S majors)

ROTC Plans

ROTC cadets are required to complete an academic plan in order to qualify for scholarship assistance and to project a graduation date for commissioning purposes. Cadets are provided these forms by their cadre. Complete a formal progress check, as recommended according to your class standing, to determine your remaining requirements, develop an academic plan, and to have your work reviewed and approved by an academic advisor.

Please note that we are unable to verify plans for prospective students.  You must be accepted and classified as an Arts & Sciences student at UHM at the time of drafting and verifying your plan for an A&S degree.

VA (VETERANS ADMINISTRATION)
Who: Arts & Sciences majors receiving VA financial assistance

Students on VA assistance (G.I. Bill) must have the courses they are taking verified for applicability to their intended degree. Please contact the VA Certifying Official in the Records Office, 808.956.8010, QLCSS room 10 for procedures and forms.
